Dental Plan Updates

Effective January 1, 2017, the Trustees of the Sheet Metal Workers Local No. 46 Health Fund are pleased to announce the expansion of the Dental Plan network through DenteMax. DenteMax provides participants residing outside of Excellus’ 31 county service area with access to a national PPO network of quality credentialed dentists, specialists and their practices with cost effective prices and savings. To find a dentist, please visit the “Find a Dentist” tab on the DenteMax homepage, http://www.dentemax.com/dentemax/home.

Beginning on January 1, 2017, claims for covered services by non-participating providers will be processed according to the Excellus schedule of allowances for participating providers, which is a different method than the UCR (usual, customary and reasonable) method used by HEG. This means that you may be responsible for larger balance bill charges when utilizing a non-participating dental provider. We encourage you to obtain dental services from a provider that participates in the Excellus network.

New Telemedicine Benefit

The new Telemedicine benefit is available to eligible participants and dependents of the Health Fund effective January 1, 2017. There is a $10.00 copay when using Telemedicine services provided through the Fund.

Health Plan Changes

The Board of Trustees is proud of the valuable benefits provided to you and your families through the Sheet Metal Workers Local No. 46 Health Fund (the Plan). As Trustees to the Plan, we continually monitor the financial stability of the Plan to ensure that the Plan will continue to provide these important benefits well into the future. In order to maintain the high level of benefits provided by the Plan, revisions to our plan of benefits are necessary from time to time.

The SMM (Summary of Material Modification) that was recently sent to you describes important changes to the Sheet Metal Workers Local No. 46 Health Fund that are effective as of January 1, 2017. These are very important changes that may affect you and your family’s coverage. If you need another copy, please contact the Fund Office. These changes will be incorporated into the next revised Health Fund Summary Plan Description (SPD).

Prescription Plan Changes

Also effective January 1, 2017, Mail Order services are available through EnvisionPharmacies. Mail order is an excellent way to receive prescriptions you will be taking for a long time with no worries about availability of supply at the local pharmacy. If you take medications on a regular basis to treat a chronic condition such as high blood pressure, high cholesterol or diabetes, mail order is mandated after one convenience fill at any contracted retail pharmacy.

Please refer to the letter you received directly from Envision on how to use EnvisionMail. You will need to obtain NEW 90-day supply prescriptions from your physician. You also must REGISTER your member information with EnvisionMail.

As of March 27, 2016, NYS Public Health Law requires your provider to electronically prescribe (Escribe) your prescription medications directly to your pharmacy. Please see this letter for details.

POMCO Update

Claim Filing Information: If you would like to submit a paper claim to POMCO, download a blank claim form from MyPOMCO.com, or obtain a form from the Sheet Metal Workers LU 46 Fund Office or this website. Complete the form and submit with any necessary substantiation detail to: POMCO, 2425 James Street, Syracuse, NY 13206, or email MyAccounts@POMCO.com, or fax to 315-703-4880.

POMCO and Your Health Reimbursement Account

Effective June 1, 2016, the Fund Office has outsourced the processing of your Health Reimbursement Account (HRA) to POMCO. Claims will no longer be accepted or processed in the Fund Office. You have received communication information and a Debit Card from POMCO. Some of the information will include:

  • "How are claims submitted"
  • "Frequently Asked Questions"

Vision Benefit

Effective March 1, 2013, your Vision Benefit changed. The Health Fund Board of Trustees decided to partner with Davis Vision, Inc . to provide an excellent vision plan for you and your dependents and we believe this is an increase in benefits for you and your family. ESI Group - Employee Assistance Program

As you know, last Fall we changed our EAP to the ESI Group. By now, you have received a cover letter, a pamphlet and an ID card. Partial Termination Benefit

Last year the Partial Termination Benefit was amended to make it easier for someone who has worked less than 400 hours in covered employment in six months, to qualify. This change in benefit has NOT been extended to June 30, 2017. You will have to have worked less than 240 hours in covered employment in six months to qualify. You may distribute up to $10,000 if you qualify.

Alternative Investment Options

Sheet Metal Local 46 Annuity Fund has added new investment options to the plan effective September 2, 2014. Ten new Manning @ Napier Retirement Target date funds were added to the investment lineup to assist the membership in reaching their retirement goals. The target date fund focuses on the approximate date when an investor plans to retire and start withdrawing their money. The asset allocation of the target date funds will become more conservative as the target date approaches by lessening your equity exposure and increasing your exposure in fixed income type investments. You should be receiving quarterly statements that will show you your investment choices, your returns and your balance. As always, it’s very important to keep your personal information updated so that you continue to receive their mailings. You can do that either directly with Prudential or through the Fund Office.

Annual Statement

It is very important to constantly monitor your pension statement and to be sure that the Fund’s records match your own. Your annual statement is usually sent in June or July. Please carefully review it for any errors or omissions you may have. It is vital that any corrections are made now as opposed to when you are applying for your pension benefit to retire.

When you decide to retire, please try to contact the Fund office approximately 3 months in advance, when possible. This will enable the process to move along smoothly and allow you to enjoy your retirement!

New Third Party Administrator

Effective July 1, 2016, Excellus will take over the duties of administering our SMW LU 46 Health Plan, including the Dental Plan. In a few weeks, you will be receiving communication information from them including new ID cards. Starting on July 1st, please use your new cards when Health and Dental services are rendered. PLEASE WATCH YOUR MAIL FOR FURTHER COMMUNICATION AND YOUR ID CARDS.

Over the past several years’ there have been substantial changes to many of our benefit plans. Regarding the Health Plan, there have been many due to the recent Health Care Reform Act. Because of that we have updated the Health section of your Summary Plan Description Booklet (SPD). If you did not pick one up at either the Union Office or the Fund Office, we have recently mailed you one. Whenever you receive updates from the Fund Office for any of the Funds, please replace them in your booklet. (Please keep the "old" one for reference purposes.)

It is very important that your family members are aware of your SPD and are familiar with your benefits. Share this very important document with them.

Please make it a point to always update your address and your beneficiaries that you have on file for all of the Funds. Life changes necessitate giving us current information.

Termination Benefit

Last year the Termination Benefit was amended to make it easier for someone who has worked less than 400 hours in covered employment in six months, to qualify. This change in benefit has now been extended to June 30, 2013. At that point in time, the Trustees will again review and decide whether to maintain the change or revert back to the original rule.

Health, Dental, and Prescription Benefits

Health Economics Group (HEG) has been our third party administrator for your Dental Claims for many years now. You should have an HEG identification card that you can show your providers. Please remind your providers to file your claim directly with Health Economics. If your provider doesn’t file your claim directly, please mail it to them at 1050 University Ave., Suite A, Rochester, NY 14607. The phone number is 585-241-9500. The extension for Dental is #501.

Lifetime Benefit Systems (LBS) and Envision continue to help us administer your health and prescription benefits.

As always, the Fund Office is available to assist you with your needs.

It is important to remember that we remain a fully “Self funded” insurance plan and as such…every penny we save, we all save together; and every penny we waste, we waste together.

2012 News Items

2012 Benefits Fair

BENEFITS FAIR: The third SMW BENEFIT FAIR was a tremendous success once again! It was held at our union hall on October 25th and 109 members and/or spouses took the opportunity to attend. The booths were manned by our professional vendors where everyone was able to get valuable information and ask questions about their benefits package. Those vendors included Manning & Napier, Prudential, EBS/RMSCO, Envision Pharmacy Manager, Aetna, Health Economics, Excellus, MVP Health Care, M & T Bank, the Pension Fund Attorney, Empire Vision, Employee Assistance Program (E.A.P.) and a Licensed Massage Therapist. Free flu vaccines were given to 72 people and 67 took advantage of the free cholesterol screening and Blood Pressure check. There were many door prizes that you were eligible for just by attending and visiting the booths. We raffled off over 30 prizes. They included a 39” and a 32” TV and many more prizes and gift cards! We’re happy to report that all comments from the members and the professionals were positive and everyone enjoyed the fair.

Thank you to all that worked hard and made this a success, especially the members and retirees that attended!!
